📜 Terms of Service

Effective Date: March 31, 2026  |  Last Updated: March 31, 2026

These Terms of Service ("Terms") are a legal agreement between you ("you," "your," or "User") and Cognizant Cloud LLC ("Company," "we," "us," or "our"), a limited liability company organized under the laws of the State of New York. By accessing or using our website at cognizantcloud.com (the "Site"), creating an account, using our tools, or accessing our API, you agree to be bound by these Terms. If you do not agree, do not use the Site or Services.

1.1 Definitions

  • "Services" means the Site, all tools, the Healthcare Data API, AI-powered features, the chatbot, and any related products offered by Cognizant Cloud LLC.
  • "Content" means data, text, information, outputs, or other materials generated by or through the Services.
  • "Credits" means the virtual currency used to access premium features within the Services.
  • "Subscription" means a recurring paid plan (Pro or Ultra) providing monthly access to premium features and Credits.
  • "Government Data" means publicly available data sourced from U.S. federal agencies, including but not limited to CMS, FDA, NIH, and NLM.

1.2 Account Creation and Security

To access certain features, you must create an account using a valid email address. You are responsible for maintaining the confidentiality of your account credentials and for all activity that occurs under your account. You agree to:

  • Provide accurate, current, and complete information during registration.
  • Notify us immediately if you become aware of any unauthorized use of your account.
  • Not share your account credentials with third parties or create multiple accounts to circumvent usage limits.

We reserve the right to suspend or terminate accounts that violate these Terms or that show signs of fraudulent activity.

1.3 Acceptable Use

You agree to use the Services only for lawful purposes and in accordance with these Terms. You may not:

  • Use the Services in any way that violates applicable federal, state, or local law.
  • Submit false, misleading, or harmful content through any feature.
  • Attempt to gain unauthorized access to any part of the Services, other accounts, or computer systems or networks connected to the Services.
  • Use automated tools (bots, scrapers, crawlers) to access the Services without written authorization.
  • Reverse-engineer, decompile, or disassemble any part of the Services.
  • Use the Services to transmit viruses, malware, or other harmful code.
  • Interfere with or disrupt the integrity or performance of the Services.
  • Use AI features to generate content that is illegal, harmful, defamatory, or infringing on third-party rights.
  • Resell, sublicense, or commercially redistribute any output from the Services without written permission.
  • Enter protected health information (PHI), Social Security numbers, or other sensitive personal data into any public-facing tool or demo.

1.4 Subscription Plans and Credits

Plans

We offer three service tiers:

  • Free: No cost, no credit card required. Includes basic tool access and 50 welcome Credits.
  • Pro ($9.99/month): Enhanced limits, 500 Credits per month, batch processing, and Deep Search.
  • Ultra ($29.99/month): Maximum capabilities, 2,000 Credits per month, GPT-5.4 powered AI, unlimited enrichment, and priority support.

Credits

Credits are consumed when you use premium features such as Magic Boost (Deep Search), Data Fixer, and batch enrichment. Credits included with your Subscription are added to your account at the start of each billing cycle. Unused Subscription Credits carry over month to month as long as your Subscription remains active. Credits purchased as one-time packs never expire regardless of Subscription status. Credits have no cash value and are non-transferable between accounts.

Credit Packs

You may purchase additional Credits in packs of 100, 500, 2,500, or 15,000 Credits. Pricing for each pack is displayed on our Pricing page. All prices are in U.S. dollars.

1.5 Billing, Renewals, and Cancellation

Automatic Renewal

Subscriptions renew automatically at the end of each monthly billing cycle. Your payment method on file will be charged the applicable Subscription fee on each renewal date. We will notify you via email before any price increase takes effect.

Cancellation

You may cancel your Subscription at any time from your account settings page. Cancellation takes effect at the end of your current billing period. After cancellation:

  • You retain access to paid features until the end of the period you have already paid for.
  • Your account reverts to the Free tier.
  • Any remaining Credits (both Subscription and purchased) stay in your account and can be used on the Free tier.

Refund Policy

Because our Services provide immediate access to digital tools and API calls, Subscription fees are generally non-refundable. However, we will consider refund requests on a case-by-case basis if:

  • You were charged in error (duplicate charge, incorrect amount).
  • The Services experienced extended downtime (more than 48 consecutive hours) during your billing period.
  • You contact us within 7 days of your initial Subscription purchase and have not consumed more than 50 Credits during that period.

Credit pack purchases are non-refundable once Credits have been added to your account. To request a refund, email billing@cognizantcloud.com with your account email and a description of the issue.

1.6 Service Availability

We strive to maintain high availability of the Services but do not guarantee uninterrupted access. The Services may be temporarily unavailable due to scheduled maintenance, unscheduled outages, third-party API downtime, or circumstances beyond our control. We provide no Service Level Agreement (SLA) for the Free tier. For paid tiers, we target 99.5% monthly uptime but this is a goal, not a guarantee.

1.7 Data Accuracy Disclaimer

The Services display data sourced from U.S. government databases and AI-powered analysis. While we make reasonable efforts to present accurate and current information:

  • Government data may contain errors, omissions, or delays in updates at the source.
  • We do not independently verify the accuracy of data from NPPES, openFDA, CMS, NIH, or NLM.
  • AI-generated content may contain inaccuracies, hallucinations, or incomplete information.
  • Data displayed through the Services should not be used as the sole basis for clinical, regulatory, or business decisions.

See Section 4 (Medical & Health Disclaimer) for additional important disclaimers about health-related data.

1.8 Intellectual Property

All original content on the Site, including text, graphics, logos, software, page design, and user interface elements, is the property of Cognizant Cloud LLC or its licensors and is protected by U.S. and international intellectual property laws. You may not reproduce, distribute, modify, or create derivative works from our original content without written permission.

Government Data displayed through our tools is in the public domain and is not subject to our copyright claims. See Section 7 (Data Source Attribution) and Section 8 (Copyright & Trademarks) for details.

1.9 User-Generated Content

When you submit queries, upload data for enrichment, or interact with AI features, you retain ownership of your input data. By using the Services, you grant us a limited, non-exclusive license to process your inputs solely for the purpose of delivering the Services to you. We do not use your inputs to train AI models or share them with third parties except as necessary to provide the Services (for example, sending queries to OpenAI for AI-powered features).

1.10 Limitation of Liability

TO THE FULLEST EXTENT PERMITTED BY APPLICABLE LAW, COGNIZANT CLOUD LLC AND ITS OFFICERS, MEMBERS, EMPLOYEES, AND AGENTS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM OR RELATED TO YOUR USE OF THE SERVICES, INCLUDING BUT NOT LIMITED TO:

  • Loss of profits, data, or business opportunities.
  • Reliance on data accuracy or AI-generated outputs.
  • Service interruptions or downtime.
  • Unauthorized access to your account due to your failure to secure credentials.

OUR TOTAL AGGREGATE LIABILITY FOR ALL CLAIMS ARISING FROM YOUR USE OF THE SERVICES SHALL NOT EXCEED THE GREATER OF (a) THE AMOUNT YOU PAID TO US IN THE 12 MONTHS PRECEDING THE CLAIM, OR (b) ONE HUNDRED U.S. DOLLARS ($100).

THE SERVICES ARE PROVIDED "AS IS" AND "AS AVAILABLE" WITHOUT WARRANTIES OF ANY KIND, WHETHER EXPRESS, IMPLIED, OR STATUTORY, INCLUDING BUT NOT LIMITED TO IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT.

1.11 Indemnification

You agree to indemnify, defend, and hold harmless Cognizant Cloud LLC and its officers, members, employees, and agents from and against any claims, damages, losses, liabilities, costs, or expenses (including reasonable attorneys' fees) arising from: (a) your use of the Services; (b) your violation of these Terms; (c) your violation of any applicable law or regulation; or (d) your infringement of any third-party rights.

1.12 Termination

We may suspend or terminate your account and access to the Services at any time, with or without cause, and with or without notice. Grounds for termination include, but are not limited to:

  • Violation of these Terms or any applicable law.
  • Abuse of the Services or API (excessive automated requests, circumventing rate limits).
  • Non-payment of Subscription fees.
  • Suspected fraudulent or harmful activity.

Upon termination, your right to use the Services ceases immediately. Any Credits remaining in your account at the time of termination for cause are forfeited. If we terminate your account without cause, we will provide a pro-rata refund of any prepaid Subscription fees.

1.13 Governing Law and Dispute Resolution

These Terms are governed by the laws of the State of New York, without regard to conflict of law principles. Any dispute arising from these Terms or your use of the Services shall be resolved exclusively in the state or federal courts located in New York, and you consent to personal jurisdiction in those courts.

Before filing any claim, you agree to attempt to resolve the dispute informally by contacting us at legal@cognizantcloud.com. If the dispute is not resolved within 30 days, either party may proceed with formal legal action.

1.14 Modifications to Terms

We may update these Terms from time to time. When we make material changes, we will update the "Last Updated" date at the top of this section and may notify registered users via email. Continued use of the Services after changes take effect constitutes acceptance of the updated Terms. If you disagree with any changes, you should stop using the Services and cancel your account.

1.15 Severability

If any provision of these Terms is found to be unenforceable or invalid by a court of competent jurisdiction, that provision will be enforced to the maximum extent permissible, and the remaining provisions will remain in full force and effect.

⚙ API Terms of Use

Effective Date: March 31, 2026  |  Last Updated: March 31, 2026

These API Terms of Use ("API Terms") supplement the Terms of Service and govern your access to and use of the Cognizant Cloud Healthcare Data API (the "API"). By using the API, you agree to both these API Terms and the Terms of Service.

3.1 API Access and Authentication

API access requires a valid Cognizant Cloud account and an API key. Your API key is personal and confidential. You must not share your API key with unauthorized parties or embed it in client-side code where it could be exposed. If you believe your API key has been compromised, regenerate it immediately from your account settings.

3.2 Rate Limits and Fair Use

To ensure service quality for all users, the API is subject to rate limits:

TierRequests/MinuteRequests/DayNotes
Free10100Basic access, limited endpoints
Pro605,000All endpoints, standard priority
Ultra12025,000All endpoints, priority processing

Requests exceeding these limits will receive a 429 (Too Many Requests) response. We reserve the right to throttle or suspend access for accounts that consistently exceed fair-use thresholds or that place excessive load on our infrastructure.

3.3 Available Endpoints

The API provides a unified interface to multiple government data sources. Current endpoints include:

  • /npi/search and /npi/lookup (NPPES provider data)
  • /drugs/search and /drugs/label (openFDA drug labels and safety data)
  • /drugs/interactions (RxNorm drug interaction data)
  • /drugs/adverse-events (openFDA FAERS adverse event reports)
  • /icd10/search (NLM Clinical Tables ICD-10 codes)
  • /hospitals/search (CMS Hospital Compare quality data)
  • /trials/search (ClinicalTrials.gov clinical trial data)
  • /recalls/search (openFDA enforcement and recall data)

Full endpoint documentation is available at cognizantcloud.com/api-docs.

3.4 Data Redistribution

The underlying government data accessed through the API is in the public domain and may be freely redistributed. However, the following restrictions apply:

  • You may not redistribute, resell, or sublicense the API itself or bulk-export data for the purpose of creating a competing service.
  • You may not present API responses in a way that implies government endorsement of your product or service.
  • You must include appropriate attribution when displaying government-sourced data to end users (see Section 7).
  • You may not cache API responses for longer than 24 hours without our written permission.

3.5 Prohibited API Uses

  • Using the API to scrape or bulk-download entire government databases.
  • Using the API to build a service that directly competes with Cognizant Cloud's core offerings.
  • Sending automated requests designed to test or probe the security of the API.
  • Using the API to process, store, or transmit protected health information (PHI).
  • Misrepresenting the source of data obtained through the API.

3.6 Service Level and Uptime

The API is provided on an "as-available" basis. We do not offer a formal Service Level Agreement (SLA) for any tier. API availability depends on both our infrastructure and the uptime of upstream government APIs. Government APIs may experience downtime, rate limiting, or data delays that are outside our control. We will make reasonable efforts to communicate known outages through our status channels.

3.7 API Changes and Versioning

We may update, modify, or deprecate API endpoints at any time. When possible, we will provide at least 30 days' notice before removing or making breaking changes to existing endpoints. We will maintain backward compatibility within major API versions (currently v1).

3.8 Indemnification

You agree to indemnify and hold harmless Cognizant Cloud LLC from any claims, damages, or expenses arising from your use of the API, including claims from your end users based on data accuracy, service availability, or your application's use of API data.

⚠ Medical & Health Disclaimer

Effective Date: March 31, 2026  |  Last Updated: March 31, 2026

IMPORTANT: The Services provided by Cognizant Cloud LLC do NOT constitute medical advice, clinical guidance, or professional healthcare recommendations. The information presented through our tools is for general informational and research purposes only.

4.1 Not Medical Advice

Nothing on the Site or in the Services should be construed as medical advice, diagnosis, or treatment recommendations. The information we display is sourced from publicly available government databases and processed by automated systems. It is not reviewed by licensed healthcare professionals before being presented to you.

Always consult a qualified healthcare professional before making any decisions about your health, medication, or treatment. Never disregard professional medical advice or delay seeking treatment because of information you obtained through our Services.

4.2 Drug and Medication Data

Our tools display drug information sourced from openFDA, RxNorm, and DailyMed. This data includes drug labels, adverse event reports, drug interactions, and recall notices. Important limitations:

  • Drug data may not reflect the most recent labeling changes, as government databases may have update delays.
  • Adverse event data from FAERS (FDA Adverse Event Reporting System) represents voluntary reports and does not establish causation between a drug and a reported adverse event.
  • Drug interaction data may be incomplete. The absence of a listed interaction does not mean two drugs are safe to use together.
  • Dosage, contraindication, and warning information displayed through our tools may not be complete or current.

4.3 Clinical Trial Information

Clinical trial data displayed through our Services comes from ClinicalTrials.gov, maintained by the National Library of Medicine. We display this data as-is. We do not evaluate, endorse, or recommend participation in any clinical trial. Eligibility criteria and trial details may change after being posted to ClinicalTrials.gov. Contact the trial sponsor or principal investigator directly for current information.

4.4 Hospital Quality Data

Hospital quality measures displayed through our tools come from the CMS Hospital Compare program. These measures reflect historical performance and may not represent current quality of care at any facility. Quality measures are one of many factors to consider when evaluating healthcare providers.

4.5 AI-Generated Content

Some features of the Services use artificial intelligence to generate responses, summaries, or data transformations. AI-generated content:

  • May contain errors, inaccuracies, or "hallucinations" (information that appears plausible but is factually incorrect).
  • Should not be used as a substitute for professional judgment or verified data.
  • Is not reviewed by medical professionals before being displayed to you.
  • May produce different results for the same query on different occasions.

See Section 5 (AI Disclosure) for additional details about how AI is used in the Services.

4.6 Provider Directory Information

Provider information from the NPPES National Plan and Provider Enumeration System may contain outdated addresses, phone numbers, or specialty classifications. NPI registration data is self-reported by providers and may not reflect current practice locations or credentials. Verify provider information directly with the provider or through your insurance plan before scheduling care.

4.7 Recall and Safety Alerts

FDA recall and enforcement data displayed through our tools reflects publicly available information from the FDA. We make no representation regarding the completeness or timeliness of recall notices. For the most current recall information, visit FDA.gov Safety Recalls directly.

4.8 Emergency Situations

If you are experiencing a medical emergency, call 911 (or your local emergency number) immediately. Do not use this website or its tools to seek emergency medical assistance.

🤖 Artificial Intelligence Disclosure

Effective Date: March 31, 2026  |  Last Updated: March 31, 2026

Cognizant Cloud LLC uses artificial intelligence ("AI") in several features of the Services. We believe in transparency about when and how AI is used. This disclosure explains which features are AI-powered, what models we use, and the limitations you should be aware of.

5.1 Where AI Is Used

FeatureAI ModelWhat It Does
AI ChatbotOpenAI GPT-5.4Answers healthcare data questions, provides guidance on tools and APIs
Magic Data FixerOpenAI GPT-5.4Cleans, standardizes, and corrects messy healthcare data (addresses, provider names, codes)
Magic NPI SearchOpenAI GPT-5.4Interprets natural-language provider queries and translates them into structured NPPES searches
Data EnrichmentOpenAI GPT-5.4 (Ultra tier)Enhances provider records with inferred specialties, practice types, and related data

5.2 AI Models and Third-Party Processing

AI-powered features use models provided by OpenAI (currently GPT-5.4-nano for standard features and GPT-5.4 for Ultra-tier features). When you use an AI-powered feature, your input text is sent to OpenAI's API for processing. This means:

  • Your query leaves Cognizant Cloud's servers and is processed by OpenAI's infrastructure.
  • OpenAI's API data usage policies apply to this processing. As of the date of this disclosure, OpenAI does not use API inputs to train their models.
  • We do not store AI conversation logs on our servers after your session ends.

5.3 Limitations and Risks

AI systems, including those we use, have inherent limitations:

  • Hallucinations: AI models can generate information that appears factual and authoritative but is entirely fabricated. This is particularly dangerous in healthcare contexts where incorrect drug names, dosages, or interactions could cause harm.
  • Outdated Training Data: AI models are trained on data up to a certain cutoff date. They may not reflect the most recent drug approvals, guideline changes, or recall notices.
  • Bias: AI models may reflect biases present in their training data, potentially leading to unequal or inaccurate results for certain populations, conditions, or queries.
  • Inconsistency: The same query may produce different results on different occasions. AI outputs are probabilistic, not deterministic.
  • Context Limitations: AI models process individual queries without access to your full medical history, insurance coverage, or clinical context.

Always verify AI-generated content against authoritative sources before acting on it. AI outputs from Cognizant Cloud are best used as a starting point for research, not as a definitive answer.

5.4 What We Do Not Use AI For

  • We do not use AI to make clinical decisions or treatment recommendations.
  • We do not use AI to determine insurance coverage, eligibility, or prior authorization.
  • We do not use AI to diagnose medical conditions.
  • We do not use your data to train or fine-tune AI models.

5.5 Human Oversight

AI-generated outputs are not reviewed by human professionals before being displayed to you. The AI chatbot and Magic tools operate autonomously in real-time. If you encounter an AI response that appears incorrect, misleading, or harmful, please report it using the feedback mechanism in the chatbot or by emailing ai-feedback@cognizantcloud.com.

Cognizant Cloud LLC is committed to honest, transparent, and fair business practices in compliance with the Federal Trade Commission Act and applicable consumer protection laws.

6.1 Advertising and Marketing Claims

All claims made on our Site about our products and Services are truthful and substantiated. We do not make misleading claims about the capabilities of our tools or AI features. Specific disclosures:

  • When we describe our AI as "powered by GPT-5.4," this accurately reflects the OpenAI model used for inference.
  • Feature descriptions on our pricing page accurately represent what is included in each tier at the time of publication.

  2. We will review your request and respond within 5 business days.
  3. Approved refunds are processed to your original payment method within 5-10 business days.

6.5 Government Data Sourcing Claims

When we state that our tools use "free government APIs" or "public federal data," this is accurate. The underlying data is provided at no cost by U.S. federal agencies through their public APIs. Our value-add is the unified interface, search intelligence, AI-powered features, and data enrichment we build on top of these sources. We do not claim to be affiliated with, endorsed by, or a representative of any U.S. government agency.

6.6 Made-in-USA Claims

Our software is developed in the United States. Our hosting infrastructure (Netlify) is based in the United States. We do not make "Made in USA" claims about any physical products through this platform.
